STEP 2 Have your child complete the activities for his or her age
group. Your child can complete the MATH activity sheet,
the READING activity sheet (or both)! Children can earn two
books by completing one activity sheet and five books by
completing two activity sheets.

STEP 3 Download the reward redemption form, print and complete
all fields. Mail the completed form and your child's activity
sheet(s) to Scholastic to qualify for your rewards!
BOOK OFFER RULES
NO PURCHASE NECESSARY. To redeem your free books, visit www.scholastic.com/kumon and download the age-appropriate activity sheets, and completely fill out and mail to: Kumon Book Offer, 557 Broadway, P.O. Box 711, New York, NY 10013. Include your name, daytime phone number, email address, complete mailing address and the ages of your children. Sending in one (1) completed activity sheet(s) will earn you two (2) free books (a "Set ") and sending in two (2) completed activity sheets will earn you five (5) books. Maximum books allowed is five (5) per household. You can only send in activity sheets if you are the parent and/or legal guardian of a child who falls within the activity sheet age groups (ages 5-6, 7-9 or 10-12). Two activity sheets maximum per age group per participant - one reading, one math. The promotion ("Book Offer") begins April 4, 2011, 12:01 a.m. EST and ends either (a) when 1,250 Sets have been awarded or (b) October 15, 2011, Midnight EST, whichever occurs earlier. Books cannot be returned.
By submitting activity sheets, each entrant agrees Scholastic shall own the activity sheet results (including all rights embodied therein in activity sheets) and that it and its designees may exploit, edit, publish, use, adapt, modify, or dispose of any entry and elements embodied therein, online, in print, film, television, or in any other media now or hereafter known for advertising, promotional, or other purposes throughout the world, in perpetuity, without additional compensation or notification to, or permission of the entrant, except as prohibited by law.

By participating, participants agree to be bound by these rules, and the decisions of Sponsor, which are final. One age group per household. ELIGIBILITY
The promotion is open only to legal residents of the 50 United States (including the District of Columbia) who are currently 18 years of age or older and who are parents or legal guardians of a child in at least one of the designed age groups set forth above.
